To be eligible to enroll, your child must be 2 to 12 years of age and be up to date on all immunizations. Once the $50.00 registration packet. The enrollment packet must be submitted once it is complete. An original immunization card must be submitted for copy at time of registration. A physical will be required for all children under age 6. All enrollment is subject to space availability and our ability to meet the needs of your child.
